GENERAL DESCRIPTION Performs highly complex (senior-level) consultative services and technical assistance work that involves planning, developing, and enhancing the agency’s family reintegration goals.
Facilitates a proactive, strengths-based, and holistic reentry experience created with, and for, youth and their families, which begins at the time of commitment and continues beyond discharge.
Work involves providing outreach and service provision to families of youth who have needs beyond what can be addressed through the agency’s standard case management and treatment practices.
Such needs include but are not limited to housing issues that could result in the youth’s potential homelessness, family reunification services to address familial discord, parenting skills classes, education for the family regarding the youth’s intellectual and physical disability, mental health issues, and assistance in navigating local community resources and social services.
Teaches assigned youth and families to navigate systems (medical, behavioral health, educational, workforce, human service agencies, community resources, housing, transportation, recreational/leisure) and to appropriately advocate for themselves within these systems.
Moves the youth toward permanency, self-efficacy and self-reliance, after discharge.
Works in partnership with the youth’s identified family members, treatment team members and assigned parole officer to provide support and enrichment approaches to bridge gaps and address areas of need identified such as: family advocacy, empowerment, academic achievement, vocational and employability skill development, and a connection to community resources and supportive relationships.
Supplements services for youth on parole provided by the parole officer to assist family members (parents/guardians) in using proactive techniques to keep youth focused on the successful completion of his/her parole, and develop and execute a plan when youth are struggling with a positive reentry.
Collaborates with parole office staff, case managers, community resource personnel, local and state mental health authorities, local and state rehabilitative service agencies, local and state child placement agencies, and private residential providers.
May train, lead, and/or prioritize the work of others.
Works under limited supervision with moderate latitude for the use of initiative and independent judgment.
May be required to have a flexible schedule to meet the needs of the campus including weekends, evenings and occasional travel.
ESSENTIAL FUNCTIONS Coordinates and manages approvals for visitation for youth and families and assists with additional support during visitation if needed to increase effective communication and provide support.
(30%) Develops and facilitates training materials for psycho education, multi-family support workshops/groups and/or skills building groups.
Provides life skills, specialized services, skills building training, parenting education and support, counseling and comprehensive case management to incarcerated and formerly-incarcerated juvenile offenders on parole status, and their families; (20%) Tracks and monitors assigned youth and assists in family services and long-term permanency planning for these youth.
Initiates and facilitates meetings between agency personnel to enhance collaboration and discuss case specific strategies for youth and families (15%) Participates and collaborates with interagency/community committees and work groups which are involved in services provided for youth and families to enhance the continuity of care and re-entry process for youth and families.
(10%) Initiates and facilitates youth specific staffing with internal and external stakeholders as a method for identifying needs and obtaining community resources for youth and families.

(5%) Raises agency personnel awareness on family dynamics, and the impact that separation and incarceration have on the family unit; addresses these issues at the family level as needed; and advocates for “family voice” within the agency.
Conducts community visits (homes, local resources centers) and coordinates transportation needs for families if needed to access mental health services.
(5%) Reports any presence or perception of racial, ethnic, and/or gender disparities.

Qualifications MINIMUM QUALIFICATIONS Bachelor’s degree with major course work in psychology, social work, family relations, criminal justice, or other related social science or behavioral science field OR Five (5) years of full-time wage earning experience that may be any combination of the following: (1) supervising or providing rehabilitation/treatment services to juveniles in a residential setting, treatment program, parole setting or correctional program; (2) experience involving development and implementation of individual rehabilitation or treatment case plans, assessment of treatment needs, development and coordination of treatment services, or individual counseling; or (3) working with at-risk families in a social services program or juvenile correctional agency.
Required: · Possess a valid driver license.
Preferred: · Master’s degree in psychology, social work, family relations, criminal justice, or other related social science or behavioral science field.
· Clinical licensure Experience and Education Substitutions: One year of appropriate experience may substitute for one year of college (30 course hours) on a year for year basis.
Completed course hours of graduate study may substitute for up to two years of experience on a basis of 12 graduate course hours for one year of experience.
Pro-rated part-time experience may satisfy the experience requirement.
Note: Any degree, diploma, high school equivalent, or course hours required to meet minimum qualifications must be from an accredited educational institution.
