*Annual Wellness Visit Nurse Practitioner * *REPORTS TO:* Chief Medical Officer and Supervising Physician *EDUCATION & EXPERIENCE: *BA/BS, graduate of AMA accredited Family Nurse Practitioner program.
Current or pending FNP license, current CPR certification.
*SKILLS:* Provide professional health care services for clients in an ambulatory clinic setting.
*PRIMARY DUTIES: * * conduct Annual Wellness Visit (AWV) visits for Medicare patients following established guidelines.
* develop and implement personalized care plans based on AWV findings.
* collaborate with the healthcare team to integrate AWVs into the overall patient care strategy.
* must have a genuine desire to continuously learn and provide high quality patient care.
* seeing ambulatory patients of the Center as scheduled and assigned by Center.
* maintain full privileges for practice of clinical medicine at Family Health Center.
* abide by the ethics of the medical profession and all federal, state and local laws and ordinances applicable to his/her practice.
* participate in committees/work groups as assigned.
* may participate in quality improvement projects.
* other duties as assigned.
*OTHER DUTIES: * * assist in the Center’s provision of health education and health promotion programs including but not limited to such groups as the Center’s patients, physicians, staff, and/or community residents.
* other duties as assigned by the Medical Director and/or Center management.
