The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems.
The centers provide campus-based direct services and supports at 13 centers located throughout the state.

Provides highly specialized and complex pharmaceutical services to improve drug usage and therapeutic outcomes for persons with developmental disabilities residing at a State Supported Living Center (SSLC).
Conducts Drug Utilization Evaluations (DUE)/Drug Utilization Reviews (DUR) on commonly prescribed medications at the SSLC.
Analyzes and documents assessments of residents' pharmaceutical regimens at regular intervals, including issues related to use of antibiotics, anti-convulsants, psychoactive medications, and polypharmacy.
Participates in Interdisciplinary Team focused on the clinical care of residents at SSLC including Health Status Team meetings and ad hoc Interdisciplinary team meetings.
Provides continuing education to pharmacists, physicians, and nurses on issues concerning drug therapy, the inherent toxicity of drugs and side effects, as well as assisting in the prescription of appropriate doses.
Serves on facility committees and workgroups which have a pharmacy related component.
Functions independently with minimum supervision and extensive latitude for use of initiative and independent judgment within the scope of practice outlined by the Texas State Board of Pharmacy.
Provide treatment to HHS employees injured in course and scope of employment.
Essential Job Functions: Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Conduct quarterly medication therapy reviews for all individuals to optimize drug regimens and provide the best possible patient outcomes.
These focused reviews include topics such as: drug interactions, polypharmacy, evaluations of laboratory values, high risk medication use, ongoing monitoring for side effects, and appropriateness of therapy.
Participate in the interdisciplinary team and use clinical knowledge to provide recommendations for drug therapy.
Interact and collaborate with professional staff from multiple departments to provide the best possible care for the individuals who reside at the State Supported Living Center.
As a member of various committees, including Pharmacy & Therapeutics, the clinical pharmacist supports the director of pharmacy to establish pharmacy’s role in the overall care of the individuals served.
Supervise activities of the pharmacy technicians, including the dispensing of medications.
Maintain open and regular communication with the director of pharmacy to ensure quality services are being provided.
Compliance with applicable Texas State Board of Pharmacy and other regulatory provisions will be the responsibility of the pharmacy director, ensuring that proper inventory controls are maintained and that pharmacy records are kept according to applicable regulations.
May develop local policies and procedures, and train associated staff members both within and external to the pharmacy department.
Participates in mandatory training per HHS policies HHS and procedures, HIPAA policies, state and federal rules and regulations, and ICF-IID regulations, and the requirements in the SSLC’s Department of Justice Settlement Agreement in order to provide quality pharmacy services and care to residents served.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
Such participation may require an alternate shift pattern assignment and/or location.
Knowledge Skills Abilities: Extensive knowledge of drug products, medical therapeutics, pharmacokinetics, and disease states.
Ability to communicate clearly in oral and written form.
Ability to work with information systems, automated, and technical equipment.
Ability to utilize Microsoft Office Suite (Word, Excel, Access, and Outlook) to prepare all written communications.
Knowledge of on-line/internet based reference materials and skills for conducting focused Literature Reviews related to the practice of clinical pharmacy.
Ability to develop and maintain liaison relationships with medical and nursing staff providing timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, etc.
Skills in conducting and participating in Drug Utilization Evaluation/Drug Utilization Reviews (DUE/DUR) or comparable medical research or Quality Assurance reviews including basic knowledge of statistics and demonstrated ability to collate and analyze data.
Knowledge of Texas State Board of Pharmacy rules, Federal Drug Administration regulations, and other comparable rules/regulations related to the practice of pharmacy within the State of Texas and CMS/HHS certified ICF-IID facilities.
Registration or Licensure Requirements: Licensed Pharmacist by Texas State Board of Pharmacy.
Initial Selection Criteria: Graduation from an Accredited College or University with a degree in Pharmacy is generally preferred, however a Bachelor of Science is acceptable.
Experience in pharmaceutical work preferred.

Flexibility in work hours may be required for this position.
The position may be required to work overtime and/or extended hours.
Must be willing to work weekends and holidays.
