*Physical Therapist Home Health Job description:*
Rehab Accomplished is a therapist-owned staffing agency serving *Hubbard, TX*, and surrounding areas.
We provide physical therapy, occupational therapy, and speech therapy services.
Our mission is to provide quality and evidence-based intervention and make a difference for others.
We would like to hire an *experienced PT *and *new Grads as well.
Training and strong support will be provided as needed.
At some point, all of us are new grads and *I believe only a therapist can understand and cater to another therapist's work needs so contact us and speak directly with an experienced therapist for further questions.
*ESSENTIAL DUTIES AND RESPONSIBILITIES*

* Evaluate patient and develop appropriate and effective treatment program in accordance with patient’s status, medical referral and therapy evaluation results while maintaining the confidentiality of all pertinent patient care information to assure patient’s rights are protected.
* Demonstrate understanding of rationale and application of therapeutic techniques, proper body mechanics, safe and efficient patient handling skills and proper equipment usage.
* Participate in clinic maintenance prevention procedures.
* Work as a part of a collaborative, multidisciplinary team with other hospital departments and physicians in order to provide comprehensive quality care to patients.
* Report to Manager patient-care management problems, equipment needs and maintenance requirements.
Cosigns all students’ notes.
Coordinate patient’s treatment with other health care professionals where appropriate.
* Plan individual patient schedule and maintain daily attendance record, daily progress notes, evaluation, home program and discharge summaries.
Charge for patient treatment on a daily basis in approved manner.
* Demonstrate compliance with department and hospital Policies & Procedures while displaying cooperative and constructive attitude in intradepartmental and interdepartmental relationships.
*Qualifications/Skills:*
* 1 or more years as a licensed physical therapist preferred.
New graduate acceptable.
* Licensed in the State of Texas.
* American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification is required.
* Master’s degree or equivalent in Physical Therapy from an accredited program.
