Summary
Works in collaboration with the CHOT Hospice Team and CHOT Chronic & Complex Care Team to provide services to hospice and palliative care patients and families within the practitioner’s scope.
The practitioner (APP) services may be provided in the various settings where the company’s services are delivered: inpatients, both GIP and inpatient hospitals; Hospice Inpatient Facilities; and various homes- personal, personal care, assisted living, and nursing.
Home visits may include new patient and subsequent clinician visits, Face-to-Face assessments/recertifications, and clinical visits, each of which will require associated documentation.
APP will perform procedures for which he/she is capably trained and privileged to do and will document such procedures and document patient assessments, evaluations, consultations, and progress notes.

Essential Functions Review clinical information and coordinate and oversee patient care in consultation with the assigned medical physician, interdisciplinary team, and other healthcare practitioners.
When indicated, consult with the patient's primary care physician and other healthcare practitioners regarding the patient's eligibility for hospice and/or palliative care, ongoing care needs and medical management related to the patient's serious/terminal condition.
Conduct in-person, telehealth, and telephone visits to assess/reassess patients’ need for Hospice and Palliative Care services as well as ongoing eligibility.
Manage health conditions and serious illnesses by developing an individualized plan of care, reviewing medications and/or treatments, obtaining consultations or making referrals and coordinating medical services.
Educate/counsel patients, families, and/or caregivers relative to preventative care, symptom management, medical problems, psychological problems, advance care planning and spiritual problems in conjunction with the interdisciplinary team to address the total needs of patients.
Participate in the Hospice and/or Palliative Care IDG meetings.
Complete required documentation to support Hospice and Palliative Care eligibility and to meet regulatory requirements for communication of findings to Community Healthcare of Texas, including documentation required for billing in accordance with Medicare and agency guidelines.
Communicate to Senior Director, Medical Director and appropriate staff regarding any patient needs requiring medical intervention.
Consult the collaborating physician when the patient's plan of care is outside standard practice and/or protocols.
Coordinate visits and scheduling with Senior Director and Team Assistant.
Maintain compliance with all regulatory and accrediting agencies Demonstrate an appreciation and understanding of the philosophy, mission, and service standards of Community Healthcare of Texas and for providing excellent customer service.
Liaison with Hospital staff and participate in any care meetings at the hospitals as requested and deemed appropriate.
Conduct face to face visits, if needed, for transfer from palliative care to hospice or for hospice recertifications.
To be teachable, humble, and open to instruction and constructive criticism.
Maintain staff privileges at necessary health facilities.
Qualifications Licensed as an Advanced Practice Practitioner in the State of Texas.
At least 2 years’ experience in geriatrics, home care, or critical care, or hospice and/or palliative care (preferred).
Excellent organizational skills.
Proficient skill using a computer is required; Microsoft office suite experience required.

Must be able to carry up to 25 pounds and lift up to 50 pounds.
Work Environment Requirements While performing the duties of this job, the employee may work in various locations (homes, facilities, hospitals, Inpatient Units) and in various conditions with possible exposure to blood and bodily fluids and to infectious diseases.
The job requires the ability to work a flexible schedule; ability to travel locally.
While performing the duties of this job, there may be exposure to outside weather conditions when traveling between sites.
Patients’ home environments are variable regarding heat, noise, air circulation and conditioning, space, cleanliness, accessibility, and all other environmental factors.
Inpatient and skilled nursing facility environments are standardized.
Office environment is in a controlled atmosphere building.
