Job Summary
As a Clinical Liaison RN, you'll build relationships with referral sources and assess patients for post-acute care.
Your duties include evaluating patients for admission to the rehab center and following Medicare guidelines for record keeping.
What You'll Do
Conduct comprehensive pre-admission assessments in a timely manner to justify rehabilitation admission.
Communicate well when coordinating admissions with referral source, patient, family, and rehab team to ensure smooth process.
Inform potential patients, their families and sources of referrals about inpatient rehabilitation and the benefits linked to the program.
Develop and nurture positive relationships with key sources of referrals.
Formulate and execute strategies to expand the market share of the main business line within the assigned hospital.
Promote BSW Rehab Center services.
Engage with community through health fairs and educational programs as needed.
Work with Director of Business Development to promote rehab services through education, events, and meetings in healthcare settings.
Ensure adherence to Medicare guidelines and maintain accurate documentation, both pre-admission and throughout the patient's rehabilitation stay.
Manage the flow of Inpatient Rehabilitation Facility-Patient Assessment Instrument to streamline processes.
Offer unmatched customer service by ensuring patient referrals are handled and executed in a timely manner.
What Will Make You Successful In The Role
Familiarity with care management and resource usage management practices.
Being well-versed with current Medicare guidelines and regulations related to inpatient rehabilitation compliance.
Understanding of established nursing and patient care standards and procedures.
Familiarize yourself with laws, rules, regulations, and standards for nursing.
This includes hospital protocols and policies for providing care.
Knowledge of medical terms, health promotion, risk reduction, and disease prevention is important.
Understanding common medications, doses, effects, and reactions is crucial.
Commitment to upholding medical and professional nursing ethics and patient privacy rights.
Excellent ability in expressing thoughts clearly through verbal and written communication.
Possessing skills related to marketing and promoting a service line.
A team player who is capable of partnering with both internal colleagues and external partners and organizations.
Good at critical thinking and problem-solving.
Basic computer skills inclusive of Microsoft Office, information security, scheduling and payroll systems, electronic medical documentation, and email.

Qualifications
Candidates should have completed an Associate level degree in the field of Nursing.
At least 2 years of relevant experience is a must.
Registered Nurse license.
